Rhinoplasty – 4
Young mid-20s male initially sought consultation for acquired nasal obstruction and difficulty breathing stemming from an old nasal fracture and displaced nasal septum. Patient could not breathe very effectively out of either nostril. And one can see that he has a deformity of the nasal cartilages with a deflection and depression on the right side. He also has an abnormal nasal bone structure on his left side. Patient’s cosmetic complaints included a more upward sloping appearing nose with a pointed tip that he wished to be corrected to be less projected. There was also the matter of the twisted appearing, flattened and deflected lower third of the nose that only an open approach rhinoplasty would correct. We were able to achieve all aspects of correction for the nasal obstruction. The septum was corrected, the internal nasal valves were corrected with internal cartilage spreader grafts, and the deformity of the nasal cartilages were supported with, dorsal grafting and tip support. The nasal bones were corrected with osteotomies and crushed cartilage was placed to give the tip appearance of a more natural rounded and less cleft-like appearance. His nose is decidedly more masculine. The nose has good balance and symmetry with the rest of his face. Most important, patient is able to pursue athletic endeavors without feeling short of breath with improved airway patency.
