This was a challenging Septo-rhinoplasty case to address this 50 yo woman’s initial concerns of nasal airway obstruction. She wanted to breathe better first, which required a formal septoplasty and inferior turbinectomy, but secondly wanted to improve the appearance of her nose to be more balanced. She had a long, wide nose with increased nostril show, and an ill-defined droop tip that was accentuated with smiling. A so-called plunging tip deformity. We corrected this with multiple maneuvers to include spreader grafts, tip sutures, cartilage grafts and depressor septi muscle translocation–a technique to diminish the tip droop when smiling. She is seen here with better definition and rotation to the tip and better length and balance of the nose for her face. Seen her almost 12 months post op.
